About the role
As a part-time Social Services Case Manager, you’ll work to facilitate client care around material and support needs with a goal of promoting a path to sustainability for your client. Work includes assessing needs, developing an Individualized Service Plan which establishes client-specific goals, and coordinating and monitoring progress. This role is approximately 20 hrs per week.
What you’ll do
- Conduct comprehensive intake assessments to evaluate clients’ needs.
- Develop goals and plan for service coordination in collaboration with the client.
- Establish and manage referrals to outside service providers.
- Create & maintain an alliance with the client to provide continuity of care and ensure compliance and progress.
- Monitor services to the client provided by other organizations and agencies.
- Maintain accurate, timely case notes and prepare monthly case reports.
- Establish and maintain appropriate boundaries, a high degree of confidentiality and ethical professional behavior.
- Meet regularly with the Social Services Coordinator to discuss client needs, service planning, and progress.
- Attend agency staff meetings, trainings, and any other required meetings.
- Other related duties assigned by the Social Services Coordinator or Management Team
Skills & qualifications
- Bachelors or Masters degree in social work or related human services field, plus 1 year of prior social service experience.
- Considerable knowledge of community organizations and resources.
- Ability to interview clients, assess effectively & provide crisis intervention.
- Knowledge of case management best practices and skills.
- Excellent interpersonal skills
- Clear, effective & professional written skills
- Can work both independently and as part of a team.
- Requires a valid Massachusetts driver’s license, current automobile insurance, and regular use of a personal vehicle.
- Bilingual ability a plus
